NSS: Annual Evaluation Form Released For Certificate Processing

The National Service Secretariat has released the Annual Evaluation Form for the 2023/2024 National Service Personnel today, August 23, 2024. This form is a crucial document for the processing of NSS Certificates for Personnel in Subvented (Public) and Non-subvented (Private) Institutions.

IMPORTANT INSTRUCTIONS

To ensure a smooth and timely processing of your certificate, follow these steps:

1. Access your dashboard to download and print your annual evaluation form.

2. To download your annual evaluation form, start by clicking “Details Confirmed by me.” This indicates that you have verified and confirmed your personal details, course of study, and institution as displayed on your dashboard. Kindly note that these details will appear on your certificate exactly as they are displayed. If you need to make any changes, you can click on “Request Changes!” to update your details.

3. Get your Head of Department or Immediate Supervisor to complete the form accurately and thoroughly after you have downloaded and printed it out.

4. Submit the completed form to the Metro Directorate/District Office for onward processing of your NSS Certificate.

IMPORTANT DEADLINE

All National Service Personnel in both Public and Private Institutions must submit their completed annual evaluation form before the end of August 2024, or as instructed by their respective Metro Directorates/District Offices. Late submissions may cause delays or, in extreme cases, result in the non-processing of your certificate.

COLLECTION OF CERTIFICATES

Once processed, certificates will be printed and dispatched to Regional Offices for collection by NSPs.

CRITICAL NOTE

All 2023/2024 National Service Personnel are hereby advised to meet the deadline for submission of their annual evaluation form. Failure to do so, as stated earlier, will result in an unprocessed National Service Certificate, leading to serious consequences for your future endeavors. Take this information seriously and act promptly to avoid any inconvenience.
